I built the forms and administration system for the Epilepsy Deaths Register, a service that collects information from people bereaved by epilepsy and from professionals. The project needed to support a substantial reporting process while recognising that people may be completing it at a difficult time.
I developed the application with Laravel, React, TypeScript and Inertia. The questionnaire is divided into manageable stages, with conditional sections, validation and clear controls for moving between them. People can save their progress and return, while the system tracks whether a record is open, submitted, expired or suspended. The experience connects the form people see with the rules and record handling behind it.
I also built the administration tools around that journey. The team can search and filter records, manage users, review reporting status and handle record reassignment and exports. A separate email-management area makes reminders visible to administrators, with status tracking and controls for reviewing or resending them.
Access control, validation and authentication are part of the application structure. Laravel handles the backend rules and permissions, while Fortify provides two-factor authentication support. The frontend uses reusable form components so behaviour remains consistent across a long, branching questionnaire.
For me, this project brings together the parts of development I care about: working through a complicated process, giving people clear ways to move through it, and building the tools the team needs to support them afterwards. It is a full application, with the reporting form and operational workflow developed together.
